The Diaphragm: The Missing Piece in Understanding Pain Patterns?

Why every bodyworker should understand this powerful, overlooked muscle

Take a moment and picture the diaphragm.

Can you picture where it attaches?
Can you imagine how it moves?
Can you feel what it would feel like under your hands?
Can you recognize when its movement is restricted or when the ribcage is not moving as efficiently as it should?

For many bodyworkers, the answer is no.

And that is understandable. The diaphragm is one of the most important muscles in the human body, yet it is often one of the muscles we spend the least amount of time learning how to assess and work with.

Most practitioners know the diaphragm as “the breathing muscle.”

And yes, it is absolutely that. But that's not all it is.

The diaphragm sits at the center of a complex system of movement, fluid pressure, and relationship throughout the body. Understanding its anatomy, mechanics, and effects on the whole body can completely change the way you look at stubborn pain patterns and recurring dysfunction.

The body does not work as a collection of separate parts--it works as a connected system, with the diaphragm in the middle of everyone involved.

The diaphragm is more than a breathing muscle

The diaphragm is a dome-shaped muscle that separates the thoracic cavity from the abdominal cavity. With every breath, it contracts and relaxes, changing pressure relationships within the body and creating movement--driving fluid movement, organ movement, etc.

That movement matters.

Movement is life!

Healthy tissues require movement. Blood needs to flow in and out. Fluids need to circulate through and around all the cells in your whole body. Organs need space to move and adapt. Joints and connective tissues depend on the adaptability of the body to create and respond to change.

The diaphragm participates in all of this.

Its movement influences the ribcage, the abdominal cavity, and the muscles that support breathing, the cranium, and the biomechanics of walking.

This means the diaphragm is not isolated.

When we understand its relationships, we begin to see why restrictions in this area may influence patterns throughout the body.

Why does the diaphragm matter to bodyworkers?

One of the biggest shifts in advanced bodywork is moving away from simply asking:

“Where does it hurt?”

and learning to ask:

“What relationships might be contributing to this pattern?”

A client may come in with:

  • recurring knee pain

  • persistent hip tightness

  • pelvic imbalance

  • shoulder discomfort that never fully resolves

  • chronic neck tension

  • jaw tension or clenching

  • difficulty taking a full breath

  • a ribcage that feels restricted or immobile

  • brain fog

It is natural to focus on the area where the person feels symptoms.

Sometimes that is exactly where the work needs to happen.

But sometimes the body is asking us to look somewhere else.

The diaphragm is connected through its attachments and movement patterns to many structures that bodyworkers work with every day. When the ribcage, diaphragm, thoracic spine, and surrounding tissues are not moving well together, strain is driven into the parts elsewhere.

The knee may not always be a knee problem.

The shoulder may not always be a shoulder problem.

The neck may not always be a neck problem.

The body is constantly adapting.

Our job as practitioners is to become better at understanding those adaptations.

The ribcage is not just a container

Many practitioners are comfortable working with muscles, but the ribcage is often overlooked.

We may think of the ribs as a protective cage around important organs, but they are also a dynamic structure designed for movement.

The ribs, sternum, thoracic spine, and diaphragm work together every time a person breathes.

They expand, and they retract.

When this system becomes restricted, the body has to find other ways to create movement.

That pulls on parts in places that seem unrelated.

This is one reason learning to assess and palpate the diaphragm and ribcage can be so valuable for bodyworkers.

You begin to see the body differently.

Understanding the body through relationships

One of the things I love most about osteopathic thinking is the emphasis on relationships.

The body is not a machine made of independent pieces.

It is an interconnected, living system.

A restriction in one area creates tension somewhere else.

A change in movement in one region can influence another.

This does not mean every problem comes from the diaphragm.

It means the diaphragm deserves a place in the conversation.

When a client presents with a pattern that does not fully make sense, curiosity becomes one of our greatest tools.

Instead of asking, “Why isn’t this technique working," we can ask, “What part of this system am I not seeing yet?” Or, "Where is this getting pulled on from?"

Why palpation matters

Anatomy knowledge is important.

But for hands-on practitioners, anatomy becomes truly useful when it moves from the textbook into your fingertips. And I'm not just talking about musculoskeletal anatomy. 

Many practitioners know what the diaphragm looks like.

Fewer have developed the ability to feel:

  • how it moves

  • how the ribs respond to breathing

  • where restrictions may exist

  • how surrounding tissues influence its function

  • how the entire system adapts

This is the difference between memorizing information and getting the information in your hands.

The more clearly we can feel the body, the more intelligently we can respond.

A different way of looking at pain

The goal of advanced bodywork is not to collect more techniques.

It is to develop better questions.

When a client presents with recurring symptoms, we want more options than simply chasing the painful area.

We want curiosity,  awareness, and the ability to recognize patterns and understand the relationships that create them.

The diaphragm is one of the most fascinating examples of this.

It reminds us that the body is always communicating--we just have to learn how to listen.
